Sex Hormone Binding Globulin (SHBG) is a protein produced in the liver that primarily binds to testosterone and estradiol, the major sex hormones in both males and females. It acts as a transport protein, carrying these hormones through the bloodstream. By binding to these hormones, SHBG reduces the amount of free, active hormones available for use by body tissues.

Total testosterone refers to the overall amount of the hormone testosterone in the body, including both the free testosterone and the testosterone bound to proteins like SHBG and albumin. It's a common measure used to assess overall testosterone production and can be a first step in investigating potential testosterone deficiencies.

Testosterone that's not attached to proteins is called free testosterone. Free testosterone acts on your tissues. Three types of blood tests measure these different forms of testosterone: A total testosterone test measures free testosterone and testosterone that's attached to proteins.
Testosterone, Bioavailable Calculated
Bioavailable testosterone is a calculated measure of the biologically active testosterone in the blood, encompassing free testosterone and that which is weakly bound to albumin. It's not directly measured, but calculated using total testosterone levels, sex hormone-binding globulin (SHBG) levels, and albumin levels, along with binding constants of testosterone to these proteins.
Testosterone, Percent Bioavailable (%)
Bioavailable testosterone refers to the portion of total testosterone that is readily available to bind to and act on tissues in the body. It includes both free testosterone (unbound to proteins) and testosterone bound to albumin, which is loosely bound and can readily detach for biological activity. The percentage of bioavailable testosterone is calculated from total testosterone, albumin-bound testosterone, and free testosterone.
Testosterone, Percent Free (%)
Free testosterone, on the other hand, refers only to testosterone that is not bound to proteins. As the “active” form of the hormone, free testosterone can readily bind to receptors and exert its effects on the body. Generally, free testosterone makes up about 2% to 3% of total testosterone.
